On the afternoon of June 15, the medical team treating at the 101-bed Intensive Care Center (ICU) located at Psychiatric Hospital (Bac Giang) conducted ECMO (artificial cardiopulmonary) intervention for a patient. 37-year-old female infected with Covid-19 .

Accordingly, female patient HTH (37 years old) was admitted to Bac Giang Lung Hospital on May 26, receiving oxygen, but her disease progress was not good. By May 28, the patient was given HFNC (high-flow oxygen therapy through a nasal cannula). On 2-6, the patient was intubated, mechanical ventilation, dialysis, antibiotics. Treating severe Covid-19 patients in Bac Giang At 8 pm on June 14, the patient was transferred to the ICU Center and continued to receive mechanical ventilation, dialysis, and vasopressor use. However, the development of heart failure was very severe, requiring high-dose vasopressors, bilateral diffuse lung damage, and did not respond to optimal medical treatment. On the afternoon of June 15, the patient was assigned to conduct ECMO placement. As the person who directly treats the patient, Dr. Nguyen Tan Hung – Deputy Head of the Department of Intensive Care and Anti-Poison (Danang Hospital), Head of the Da Nang Medical Delegation in Bac Giang Hospital – said that after placing ECMO, vasopressors were discontinued and the blood oxygenation index improved. However, the prognosis for this 37-year-old female patient with Covid-19 is still severe because of damage to both her heart and lungs. Previously, on June 12, at this center, ECMO was also placed for NTT patients (67 years old). The patient tested positive for SARS-CoV-2, then was admitted to Bac Giang Lung Hospital for treatment from June 3 with shortness of breath, cough, SpO2 85%, mechanical ventilation and dialysis. Medical facilities are having 117 Covid-19 patients with severe prognosis On June 12, the patient was transferred to the ICU in a very serious condition, unresponsive to the ventilator, so he was intubated, mechanically ventilated, but the patient’s lungs were badly damaged and spread to both sides. The patient was placed by the team of Danang Hospital and medical staff within the same day. According to Dr. Hung, the patient is now awake, still dependent on ECMO, and his lungs are on an improving trend. It is known that as of the afternoon of June 15, there are 60 patients at the 101-bed ICU Intensive Care Center. In which, the good news is that more than 25 patients have been successfully weaned from HFNC; Eight patients tested negative for SARS-CoV-2 for the first time and two patients had a negative test for SARS-CoV-2 for the second time. According to the Treatment Sub-Committee, the National Steering Committee for Covid-19 Prevention and Control, so far, there are 117 Covid-19 patients with severe prognosis in health facilities nationwide, 52 cases with very severe prognosis, and 6 cases with severe prognosis. case is at risk of death. There are 13 cases of ECMO intervention in the whole country, especially in Ho Chi Minh City, there are 4 serious and critical cases requiring ECMO intervention. Assoc. Prof-Dr. Luong Ngoc Khue, Director of the Department of Medical Examination and Treatment Administration, has sent an official dispatch to the Department of Health of Ho Chi Minh City on actively strengthening the management and treatment of Covid-19 cases and daily case reports. . To ensure the best management, care and treatment for patients, the Treatment Subcommittee (National Steering Committee for Covid-19 Prevention and Control), directs medical examination and treatment facilities to focus as much as possible. all resources to prioritize management, treatment and comprehensive care for Covid-19 cases.
HTH female patient, with a healthy history, no underlying disease, was found to have Covid-19 on May 26, transferred to Bac Giang Lung Hospital for treatment, had to breathe oxygen but her shortness of breath did not improve. . 2 days later, the patient was transferred to non-invasive mechanical ventilation. On June 2, she continued to be intubated, mechanically ventilated, dialysis osiris, and used antibiotics. Dr. Hung and colleagues intervened ECMO for patients at the 101-bed ICU Center in Bac Giang Due to her worsening condition, on the evening of June 14, Ms. H. was transferred to the largest Intensive Care Center (ICU) in the North with 101 beds located at Bac Giang Mental Hospital for mechanical ventilation, dialysis, and use. vasopressor drugs. However, the development of right heart failure was very severe, requiring high-dose vasopressors, bilateral diffuse lung damage, and did not respond to optimal medical treatment. Therefore, at 4 pm on June 15, the patient was assigned to place ECMO (artificial cardiopulmonary support). Dr. Hung said that after placing ECMO, the patient stopped taking vasopressors and improved the blood oxygenation index. However, the prognosis remains severe due to damage to both the heart and lungs. In this case, it will be necessary to continue to closely monitor each indicator. Earlier on June 12, also at the 101-bed ICU Center, doctors of Da Nang Hospital coordinated ECMO intervention for a 67-year-old female patient NTT., 67 years old. This case also had a history of being perfectly healthy. The patient tested positive for SARS-CoV-2, then was admitted to Bac Giang Lung Hospital for treatment from June 3 in a state of shortness of breath, cough, and blood oxygen reduced to 85%. enter and filter blood. At 12 o’clock on June 12, the patient was transferred to the 101-bed ICU Center in a very serious condition, intubated, and mechanically ventilated, but the patient’s lungs were severely damaged and diffused on both sides, so ECMO intervention was indicated. . With this case, up to now, according to Dr. Hung, the patient is awake, still dependent on ECMO, and the lungs are tending to improve. So far, the country has had 37 critical patients, of which 13 had to undergo ECMO intervention, of which the Central Hospital for Tropical Diseases and Tropical Diseases Ho Chi Minh City had 4 cases each. Thuy Hanh

According to information from the largest intensive care center in the North, female patient HTH (37 years old) admitted to Bac Giang Lung Hospital on May 26 received oxygen, but her disease progress was not good. On May 28, the patient was given HFNC breathing. On June 2, the patient was intubated with mechanical ventilation, osiris dialysis, and used antibiotics. At 20:00 on June 14, the patient was transferred to the 101-bed ICU Intensive Care Center and continued to receive mechanical ventilation, dialysis, and vasopressor use. However, the state of right heart failure is very severe, must use high-dose vasopressors, diffuse bilateral lung damage, does not respond to optimal medical treatment. Therefore, at 4 pm on June 15, the patient was assigned to conduct ECMO placement. As the person who directly treats the patient, Doctor Nguyen Tan Hung – Deputy Head of the Intensive Care and Anti-Poison Department (Da Nang Hospital) – Head of the Da Nang Medical Delegation at Bac Giang Hospital said: “After placing ECMO , the patient was able to stop vasopressors and improve the blood oxygenation index. However, the prognosis remains severe because of damage to both the heart and lungs. Previously, on June 12, at the 101-bed ICU Intensive Care Center, ECMO was also placed for NTT patients (67 years old). The patient tested positive for SARS-CoV-2, then was admitted to Bac Giang Lung Hospital for treatment from June 3 with shortness of breath, cough, SpO2 85%, HFNC breathing and dialysis. At 12 o’clock on June 12, the patient was transferred to the 101-bed ICU Center in a very serious condition, unresponsive to HFNC, so he was intubated, mechanically ventilated, but the patient’s lungs were severely damaged and diffused. On both sides, P/F is lower than 60. The patient was ordered by the team of Danang Hospital and friends to place an ECMO on 12/6. With this case, up to now, according to Dr. Nguyen Tan Hung: “The patient is awake, still dependent on ECMO, the lungs are tending to improve, wish all the best for the patient”.

Today, the ICU (ICU, Central Hospital for Tropical Diseases) where I work officially surpassed the milestone of receiving 50 severe and critical COVID-19 cases in 41 days of the fourth pandemic wave.
Over the past 41 days, it’s really appreciated that we continuously receive so many favors.

The sisters said to me: We can do everything to share with you, only we can’t rush in to treat patients together.” . Just hearing that, we felt so grateful. In the past 41 days with over 50 critically ill and critical patients, our ICU has never been so “epic”. Patients spread from area 1 to zone 2, a total of 50 ICU beds. Something is missing, the ICU has it. We live and work with a number of “assets” of up to many billions of dong: High-function ventilators up to more than 50 “em”, ECMO to nearly a dozen staging, Prismaflex to nearly 20 “em”; The artificial kidney machine also reached nearly 20 “em”. We go to bed also worry about the machine… scratch the paint. About 41 days, the ICU alone consumed 150 Oxiris filters, 50 sets of M100s, 10 sets of ECMO and 2000 bags of Prismazon. Thought I saw… terrible! *** So more than a month has passed, time flies. Today, the ICU house is as crowded as a festival, busy and chaotic. There are joys, but many sorrows. This is probably the most memorable time of our lives. Colleague love, comradeship, love, humanity are all here. The pressures and hardships we have never dared to complain or complain about, because I know that our hardships are just a small corner in the fierceness of the COVID-19 pandemic. We here only know how to do our best, wholeheartedly to save the critical COVID-19 cases. And, in the past 1 month, over about 4,000 new cases of this fourth outbreak, many of which are severe, fast-paced, and critical. Many patients are very sick, but have escaped death spectacularly. Tracheostomy for COVID-19 patients at the ICU bed. One thing I am sure of is that my female colleagues have shed tears at least once in the past 41 days. Because they are not the same person, they are also weak and weak. They have an average weight of less than 50kg, a height of less than 1.6m, but their strength is extraordinary and their love is intense. They love their job, love people, love all of this life. They fight with love for their profession, with their duties and responsibilities as doctors. They have overcome many hardships and challenges, although everyone faces the risk of infection and the work intensity is terrible. I love those girls, the golden girls in the COVID-19 village. Small but strong, weak but indomitable. The pandemic is so fierce, we are just a small corner in this pandemic. Do not dare to be pompous. But everything we experienced is very difficult to describe in words. Female friends can burst into tears. Who can bear to think of a baby crying all night because of thirst for milk, when a loved one has to be separated without a date to return, this tear cannot be swallowed. Who can be at ease when the whole family goes to fight the epidemic, leaving a sick and lonely mother in a 12m2 motel. And who can rest easy when both parents rush to the front line, leaving the child bewildered all night. We have chosen to sacrifice many things to contribute a part to the great cause. And male medics behave their way. They ran down to the yard, screaming like a man out of control. What is he shouting, you know? “50 patients! 50 patients! Fight to the end! Fight to the end!” (50 patients related to COVID-19 have died, by noon on June 15, the number of patients who died was 61). God, I felt my eyes go away, and I wanted to hug him and cry. That is the person who always volunteered to bury the ill COVID-19 patients, who had no loved ones by their side at the time of death. Please adhere to 5K and cherish precious vaccines Life is inherently impermanent, leaves fall to the roots. Those unfortunate people, they died without family, but we, the doctors, the nurses were always with them, holding their hands throughout the battle. With a mortality rate of less than 3%. If forced to choose to meet the amount of that 3%. I firmly believe those people, people who have reached an age that is rare, who have tasted all the bitterness of life, who have struggled with cancer. They will be willing to accept the departure. to leave the lushness of life to flourish. Giving life to a 25-week-old baby, giving up the happiness of a couple’s love to a 27-year-old boy, giving up the dedication to science of doctors, or simply giving up smiles to children… They know that we have put our heart and soul into pulling them out of the gate! They knew that we had spent the last 41 days up all night beside them, they knew that the doctor’s eyes were wilted from lack of sleep. Today is the 41st day, I don’t know how many long days and nights will be left, we have to fight the epidemic and win against COVID-19. No one is allowed to fall. We want people to adhere to the 5K, to appreciate the precious doses of vaccines. Get vaccinated if you can, and support the Vaccine Foundation if you can afford it. Let’s join hands to fight the pandemic and spread love. Me and my friends are proud to be soldiers in white at the heart of this storm… Nurse Nguyen Thi Thuong ( Chief Nurse, Intensive Care Unit, Central Hospital for Tropical Diseases )
