MSc. Nguyen Duy Huan said, Ngan is among over 10% of the population with congenital rhinoplasty, but 4 previous surgeries, doctors did not find out. In these cases, if the nose is only raised, a short time later will be skewed, even causing difficulty breathing.

Because the patient’s nose has been corrected too many times, the curvature of the septum, the nose axis and the tip of the nose are all deviated, so using only silicon or ear cartilage can not completely overcome.

Therefore, Dr. Huan had to chisel the bones on both sides, form a new bone bond, then combine using rib cartilage to erect the nasal pillar and tip, reconstruct the septum, use silicon to push the bridge of the nose higher. The surgery lasted 3 hours. Currently, after more than 1 month of surgery, Ngan’s nose is back in balance.

Dr. Huan shared, the patient was taken 6cm of rib cartilage number 6, right the right breast line. Then the cartilage is cut into thin slices, grafted to the nasal pillar, the tip of the nose. Due to the material transplant itself, the patient does not worry about rejection.

Which rhinoplasty material is optimal?

According to Dr. Huan, Asians in general have a short nose, flat nose, wide nose wings. Therefore, many people look to plastic surgery to correct the tall, more elegant nose, and the nose tip is more fluttering.

Usually, to lift the nose, depending on experience, doctors can choose artificial cartilage (silicon, ePTFE, medpor) or autologous cartilage (coronary cartilage, rib cartilage, dermis). The most common is the use of man-made materials, but it can be eliminated or shrink over time. When having to repair it will be very difficult to look beautiful, in case the tip of the nose is exposed or infected, it will leave a scar that is difficult to repair.

To make the tip of the nose tall and long, 10 years ago, doctors started using nasal septum cartilage to extend the tip of the nose, but Vietnamese and Asians in general have weak and little septal cartilage, so it is not enough to help. the tip of the nose is slimmer.

From 2015 onwards, this method has been rarely used. Then doctors in Korea, Japan and some European countries use rib cartilage (usually take the rib cartilage number 6 -7) instead.

“Since ancient times, doctors cut the rib cartilage and transplant it on the bridge of the nose, while the rib cartilage needs to be fed through vascular osmosis, so this method is ineffective, the aesthetics is not high due to warping. Over time, the current rib cartilage slicing techniques have been very ingenious, thin slices, which can be grafted into nasal pillars, septum, and tip and used in combination with other materials, so the efficiency is very high ”, Dr. Huan shared.

The image of rib cartilage is sliced ​​thin, then it will be trimmed to suit the nose shape of each patient before transplanting.

Depending on the patient, can take from 4-6cm rib cartilage. The skin incision at the site of the rib cartilage is very short, about 2-3cm, if the scar is taken care of, it is very fuzzy, so the patient does not need to worry about aesthetics. In particular, rib cartilage is a material in itself, so it is not worried about being eliminated over time. The loss of cartilage is very short, so it does not affect the health and movement of the patient.

According to Dr. Huan, the method of using rib cartilage is especially suitable for patients with deformed nose due to malformation of cleft palate, cleft palate, deformed after trauma, nose shrinkage due to complications of multiple rhinoplasty or infection. sore nose …

Dr. Huan shared, each material has its own advantages and disadvantages, in rhinoplasty there are 6 groups, depending on the shape of the nose, the wishes of the patient, the doctor will choose the combination of materials for optimal efficiency. Best:

Group 1: Those who already have a high nasal bridge, who want the tip of the nose to be longer, can just lift the tip of the nose with the coronary cartilage.

Group 2: The patient has a low nose shape, using a nasal lift using artificial cartilage made of silicon or gore-tex (material from e-PTFE resin, softer than silicon), combined with covering the tip of the nose with autologous cartilage (ear cartilage , temporal balance, fat mesoderm).

Group 3: Using artificial cartilage (silicon / ePTFE) to lift the bridge of the nose, the tip of the nose uses artificial mesoderm material (alloderm / megaderm).

Group 4: Must intervene in the septum using TnR Mesh material (is a biodegradable polyester) or Medpor, used in combination with artificial cartilage lift (silicone or gore-tex).

Group 5: Using artificial cartilage to lift the bridge of the nose (silicone, gore-text), combined with rib cartilage to make the septum and tip of the nose.

Group 6: Use whole cartilage itself, rib cartilage or a combination of fatty mesoderm and rib cartilage.

Dr. Huan warned, capturing the beauty of many women, there are many spas, hairdressers also advertise nose lift.

“Because the workers do not have surgery expertise, they often make the wrong incision, there is a straight incision in the tip of the nose or the incision on the nasal cylinder, making the post-implant material exposed”, Dr. Huan shared.
